They are tested, first on animals, then on people. During the human tests, ALL medical conditions that happen during the trial are recorded. You really can't tell if that headache someone got while trying out new drug was a side effect of the drug or just a headache. So, if you try out an ED drug on say, 1000 men, and 4 of them get satyrisis (one of the horrible conditions they mention), they still have to list that as a potential side effect of the drug. The other 996 men can take that drug for the rest of their lives and probably never have to worry about that particular problem, but it still gets picked up as a side effect.
If you did the same thing with marijuana, a clinical trial would show that it causes memory loss, gynomastia, and chronic respiratory problems. Will most people who use marijuana exhibit those symptoms? No, but if only 1 or 2 did, those would still be the listed side effects for "approved, medical marijuana".
However, you do bring up a good point. The real reason marijuana is not legal is because of a smear campaign run back in the 1930s, and because it never became a commonly used recreational drug until the 1960s. Because of the legal ban against it, physicians have not been keen to prescribe it, so it lives in a sort of medical drug ghetto, where it is treated as being much more dangerous than it really is.
Just remember, if you're going 6 miles over the speed limit, a police officer will speed 20 miles over the limit to catch you, write you a ticket, then tell you that it's "for your own safety". Same thing, really.
