Megumi D replied: "IF you have bipolar d/o, and you are also taking Effexor (or any other SSRI antidepressant such as Prozac, etc.), it can cause you to go into an agitated state, especially at your age. You might need an "atypical" antidepressant, such as Wellbutrin, for the depressed phase of your b/p d/o, because they have less of a tendency to cause the agitation than the SSRIs. I am NOT in a position to prescribe meds for you, because I am not treating you, but it does sound as if you need another medication, a "neuroleptic", such as Abilify, Risperdal, etc., to help stabilize you and decrease the agitated, angry mood. You may need another mood stabilizer (which is what the Lamictal is) such as lithium or Depakote, or an increase in your dose of Lamictal to help stabilize you again. A lot depends on what has been tried before, your personal health, before anyone can say for sure, even your psych MD. With b/p disorder, unfortunately, medications that worked well for a while sometimes stop working or need to have the dosage adjusted, for reasons that are not all clear.
The doctor who prescribed your current medications needs to know the effects of the medications on you. What you are describing is NOT what s/he had in mind to do when you were given the meds! As you know, if you have b/p, it is typical for it to "swing", and that's what's happening now. It COULD very well be that the Loestrin is causing the fluctuations--hormones can definitely affect the meds as well as your disorder. Unfortunately, the only person who can tell you for sure, or at least with a higher degree of confidence, is a psych MD to treat you, who prescribes the meds in the first place or a psych MD that you should see regularly for medication monitoring. S/He should be informed of your situation right away. No one here, not even the doctor who gave you the meds, can be precise about what is going on for you without seeing you in person.
I hate to add it to the list of people who have said it, but you do need to see a doctor. If you don't have a regular psych MD that you see for the bipolar d/o, you definitely should try to get one. They can make all the difference between being able to live a relatively stable life and going through the excesses that you are experiencing now. I know this not only as a clinical psychologist but as the mother of a 10-year-old son with bipolar d/o. It can be a very difficult disorder to treat but it IS possible to become stable on medications without a lot of bad side effects too.
I really hope that you can get someone to help you through all of your medication problems. You deserve to live a more stable, happier life and not have all of these problems, which only make you feel worse about yourself."

Bipolar is a potentially fatal illness-you're messing with dynamite here to not get help right away. I'm thinking the loestrin could have aggravated things, the timing is awfully suspicious, but bipolar can whack out on its own, too. When i have been in your situation, it often escalates to a psychiatric hospitalization, which costs a lot more than going to the doctor (unfortunately, I've never gotten the bipolar under good control). Soooooooo I'm going to repeat what the others have said, this is something you need to go find a doc. In the long run, it is a mistake to have bipolar disorder and not have a psychiatrist. Bipolar is a very serious disorder.
PS when you are feeling better again, make sure you apologize to bf and mom!! I hate when these things happen, but when you are bipolar you have to learn to apologize frequently and sincererly."

M W replied: "If you have any kind of insurance at all (even through your family), you can go to the emergency room and ask to see a psychiatrist. (You can even do this if you have no insurance at all -- they HAVE to help you.) Tell them you are having an adverse reaction to psych medication and need to see someone right away.
This is why your gynecologist should have referred you to a real psychiatrist instead of perscribing the medication herself for you. A real shrink would be reachable right now and tell you what to do. (Doesn't your GYN have an answering service that will page her for you?)
As with anything, if you can't get ahold of any of your regular docs, you can always go to the ER. Don't be afraid to ask for help.

Is this lifestyle safe? (Need a doctor)? Alright I have been feeling extremely exhausted lately. My new job is a bar back at a bar/lounge. The place gets packed so I have to constantly be moving. But this bar is half outside, in the Phoenix heat at around 110%. So I'm running around at 4 p.m. until 1 a.m. The only time I break is when I eat for 10 mins and when I use the restroom. This place isn't really strict its just busy as hell and I want ot keep my job.
Also, I'm on anti depressant effexor XR. I'm taking vicodin at about 2 pills every other day from a prescription I got for my wisdom teeth extraction a month ago. Then I am smoking herb practically every day.
So I go to bed at 4 a.m., wake up at 2 p.m., go to work again at 4, get home at 2, repeat.
I eat well overall. I take fish oil, vitamins occasionally, drink lots and lots of water, stretch, and that stuff.
But I feel exhausted constantly. I work 40 hours a week on average for the past 4 weeks. All night closing shifts.
Am I going to run out?
I am 18 years old. Just moved down to Phoenix from Prescott so I could move into my dads and work. I used to eat, sleep, and excersize but the past month with work I have irregular eat schedules, diets, drugs, anti depressants, alcohol. shifts, sex, and more.
Yeah I'm not being very honest but I can't cram this shit into yahoo and I don't want to waste peoples time.

Another factor could be the weed/herb your are smoking and the vicodin which is actually a narcotic (see second website)
Have you stopped to consider that you may be having interactions with all of these things which are making you tired? Plus the shift work you are doing is probably not helping either. Another factor which you do not mention is what age you are - if you are young maybe you will be OK - but perhaps you need to consult a health professional and be frank and honest with them as to your choices in coping medications"
How can I improve my concentration ? seems like iv tried everything sources have told me i'm 21 years, healthy, i take a high potency multivitamin, extra vitamins c, b 12 and E along with acetyl l carnitine, and alpha lipoic acid EVERY FREAKIN DAY and yet i still have attentional problems, i also try and get 30 min of exercise daily. my medications aren't helping much either... i have social anxiety and i take effexor Xr at 225 mg (targeting serotonin and norepenephrine) and seroquel. yet i simply can't focus on a task, it's so annoying, like when i read something i don't read i skim looking for things that seem important but i can't actually focus on the task, i also have a difficult time following conversations at times, sometimes it's hard just to get my thoughts out whenever I have to speak. i do spend a lot of time thinking, ruminating and introspecting, do you think it has something to do with that? am i thinking too much? what's going on? somebody please explain.

It is often harder to concentrate in a bedroom since that is a resting place, so try to avoid that as well. I'm 22, and I have similar issues especially when it comes to competing course work. I have found that for me, forcing myself to go to the library works wonders for completing work type tasks. As far as other activities that are not reading based, like conversations, try to find ways to really engage yourself in the conversation. Perhaps trying to visualize what is being talked about could help you to stay concentrated on the topic at hand. Make more eye contact and try to clear your thoughts of what is going on in your surroundings. I know it's difficult to do at times, but I think this is something that you can gradually improve upon. :)"
